The first and foremost goal of ‘Health People 2010’ is “Increase Quality and Years of Healthy Life.” With reference to this goal, I have chosen to summarize the facts mentioned in the focus area ‘Heart Disease and Stroke’ in this article.
The goal of Healthy People 2010 is to improve the health and also the quality of life through means of early detection, prevention and also treatment of risk factors that lead to heart disease and stroke. The goal also includes to identify and treat heart attacks and strokes in early stages and also to prevent recurrence of cardiovascular events.
The target number for reduction of deaths due to coronary heart disease is 166 deaths per 100,000 population. In the year 1998, this number was 208 and now 20% improvement is aimed at.
Objectives which help in achieving this are developmental increase in the proportion of adults who are aged more than 20 years and who can detect early warning signs and symptoms of heart attack and thereby call for emergency help 911, increase in the number of people with heart attacks who receive appropriate artery-opening therapy.
Another important objective is to reduce the number of old people from getting hospitalized for congestive heart failure as the principal diagnosis.
The target for stroke is 48 deaths per 100,000 population when compared to 60 deaths in 1988 (20% improvement). This target is expected to achieve by increasing the proportion of adult population who are able to identify early warning signs and symptoms of stroke.
The target for high blood pressure cases is 16% for population aged 20 years and above. This is from the baseline target of 285 in 1988-94. The method of target setting for this objective is “better than best.”
Other important objectives pertaining to blood pressure are to control blood pressure in atleast 50% of adults with high blood pressure, to make atleast 95% adults with high blood pressure to take some form of action in the form of either losing weight, reducing intake of salt or increasing physical activity or all of these and also to get atleast 95% of adults aged above 18 years of age to get their blood pressure checked in once in 2 years.
The target for mean cholesterol levels in adults is less than 199mg per dl. The objective pertaining to this target is to reduce the proportion of adults with high cholesterol levels in the blood to 17% from the baseline of 21% and also to motivate atleast 80% of population above 18 years of age to get the blood cholesterol level checked once in 5 years. Another important objective is to decrease the LDL cholesterol levels to less than or equal to 100mg/dl.
Leading health indicators
The leading health indicators which are useful to discuss with reference to heart disease and stroke are physical activity and obesity.
The objective pertaining to physical activity are to increase the number of adolescents who engage themselves in cardio-respiratory fitness physical activity for atleast 3 days a week with 20 minutes per occasion and also to increase the number of individuals to engage in atleast 30 minutes of physical activity everyday.
As far as over weight is concerned, the objectives are to reduce the proportion of adolescents, adults and children who are obese and overweight.
‘Healthy people 2010’ has specific goals and objectives for heart disease and stroke the leading health indicators for these conditions are physical activity and obesity.
